Just been sent another esa 50

Hello, I have just been sent another esa50 form to fill out , i only had an assessment in October 2015 ,and was put in the support group from wrag group , My question is why have they sent me a another esa 50 so soon ? how long does the award last for ?

When you received the award letter in 2015 did it have any dates saying from when to the time you may get any more...response from them.
