Doctors letter for UC migration

Hello

I've posted on here before about that my husband and I have to transfer over to UC (I have mild CP and he's my carer) and I know I have to ask for a Doctors note for when we sign up. I don't know exactly how to go about that other than making an appointment. I've not had to do this for years so I don't know if it has to be in a certain format, certain things said etc. Any help would be appreciated.

    If you're currently in the ESA Support Group, you shouldn't need a fit note when migrating to UC.

    It does seem to be requested automatically, so you may just need to put a message in the journal if they ask for one.

    If you need a fit note for any other reason, they can often be requested through your GP's online system without having to make an appointment now.

    I see from a previous thread you are currently claiming Income Support.

    For the avoidance of doubt, it’s a fit note (formerly known as sick note) that you need, not just a note. Just contact your GP and ask for a fit note. Anything can be put on a fit note, for example, you have CP so they could put that on it.

    Thank you. My husband is the one claiming IS alongside his CA and because we're married we both have to migrate to UC, I'm only on PIP.

    So I'll just have to call and ask for a fit note?

    I'm on PIP and my husband is on Income Support and Carers Allowance, we're both being asked to migrate onto UC because of the rules regarding if you're living with someone either common law or married you both have to make a joint claim.

    I know they'd probably ask for one but as I've been told on here that it's best to get one before applying so it's less stressful I'm going to do that.

    It's not just your husband that's claiming IS, you will also be named on that claim. It's a means tested benefit so you would be claiming as a couple.

    Yes, just speak to your GP to ask for the fit note. Please don't rely on UC asking them for this because that's not what they do. It's your responsibility to provide the fit notes and to continue to provide them without any gaps until a decision is made on the work capability assessment.

    Oh I thought you mean separately before, sorry! Do you know how long each fit note lasts? Or does it depend on how long the WCA decision takes? Sorry to be such a bother (>_<')

    You're not a bother, please don't worry about that. A fit note lasts for as long as they provide it for. The longer it's for the less amount of times you need to ask for another. For this reason, I'd advise asking for as long as possible. If it does run out before a decision is made you'll need to provide another.

    Hi,

    I'm in a similar position, need to ask doctors for a fit note (haven't seen a doctor in a long time) and not sure what to put on the request? can i ask what you put in your request? can you request how long its for?

    Put whatever health condition/s you have. When my daughter first claimed UC back in 2021 her GP put ASD and learning disability on hers. Her fit notes were for 6 months at a time.

    @summersunflower yeah I asked for mine yesterday, a Dr called me back asked questions etc and sent me it via email.

    hi there I wonder is someone can tell me , I am I the position and get carers allowance, pip, and income support.
    I applied due to managed migration and have been asked to supply a fit note. Which being a carer thought I did not have to.

    I had a fit note for pip which was indefinatly years ago

    My doctor gave me a new one , he gave indefinatly but only put the date I was assessed, which is 5 days after my claim as I had to wait for gp app.

    He did not put all my conditions down just three

    Does the Dr need to state all of my conditions?

    Also does my Dr have to put the start date to the day I made my claim?

    Please can some tell me
