where do you send the forms back to Belfast or Wolverhampton

    Hey @greyhound65 your forms should already have the address on them and should've included an envelope?

    it did not come with envelope

    The address should be at the bottom of the form you received. Addresses are different for everyone depending on where you live.

    why I ask the envelope it came was sent from Belfast but on the back of my form it address for Wolverhampton

    Yes, they are sent from a different location. It's the address on the form you need to send it to.
