Basic standardised assessment of functioning for a teenager with CP?

Hi there
I'd like to know if anyone has any experience of a good basic standardised assessment of functioning for a teenager with CP? I'm an Occupational Therapist about to start working with a teenager and would like to select the right assessment to allow me to get a good baseline picture of this person's needs. Thanks very much.
