Our car is registered in my wife's name and has disabled tax.I am the named driver on our insurance and am also disabled.Can I use the car for my hospital appointments

No not for your own personal Hospital appointments.
The vehicle must be registered in the disabled person’s name or their nominated driver’s name.
It must only be used for the disabled person’s personal needs. It cannot be used by the nominated driver for their own personal use.

Unfortunately it is quite clear that the car can only be used for the disabled person's needs. Unless there is a personal reason for your wife to be there at the same time you cannot use the car for just your own needs.
