PIP form template
Hi All,
Is there a template of the PIP form for first time claiming the benefit that can be printed out and written on in preparation for filling out the real form?
Thank you in advance.

Hi @dannistar, There's an example form on the government's website that might be helpful to prepare with:
Personal Independence Payment - How your disability affects you
Hope that's useful in some way 😊

Hello
My daughters DLA says til 20/5/2028 but will PIP forms come before ? Or do we apply?
with our sons DLA we had invite to PIP automatically (he’s 20 now) and then his carer spoke on phone to someone who asked a few questions , we filled out form & he was awarded PIP but I wonder if it’s changed as that was Covid times around then.
Any kind responses are very welcome please, I know I have ages but my mind & OCD is heightened at the moment0 -
Hi @dannistar
The process you went through with your son is still the same so she'll receive an invitation to apply for PIP when she's approaching her 16th birthday😊1 -
Ok thank you.
