Condition worsened...do I tell Pip

Hi, I'm looking for other's advice/experience?
I've been on Pip daily living for 5yrs or so. In 2018 things got worse (athritus spine), I told Pip, I had to have a new assessment and they decided I wasn't entitled to anything. I don't know what happened...i asked for a review but no decisions were made etc and finally 19weeks later, someone said, oops sorry, here it as and you now get mobility. 
Things have gotten much worse...I now have to self catherterise and I have secondary fibromiagia which leaves me chronically tired and in pain. On the pip score matrix I'm way higher than I was. But my bad experience before means I'm too worried to tell Pip. Anyone had promising storyline?

    Doing a change of circumstance always carries a level of risk. I always advise people to get expert help in regards to making changes as you potentially could put your current award in jeopardy.

    The only experience I can share with you is that I also have fibromyalgia so I can only empathise with you.
