Video Call Appointment without sending back “How your disability affects you” form

Hello, I really hope everyone is well.
I have recently applied for PIP and was sent the “How your disability affects you” form.
I didn’t get to fill it out and send it back because I struggle with my mental health. It makes me very anxious.
Despite this I have received a text from Independent Assessment Services about a video call appointment they have set me.
Is this normal? Do I still need to send the form back before the appointment?
Any help/comment would be much appreciated! Thank you.

It's rare but it does happen. If you have a marker on your file because of your health condition then they can proceed with your claim without returning your form. The only issue that can be caused by this is they won't have any information about your health condition. You don't have to return the form before the assessment if you don't want to.
I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.If i see a question that i know the answer to i will try my best to help. -
