Help with MR

Hi All 
I need to put in a mandatory reconsideration for my pip claim. I didn’t realise they took all the details over the phone. I’m claiming due to the affects of a brain bleed. I prepared myself so much for the assessment phone call the assessor noted I had no cognitive impairment and good memory. I have spent the last two weeks recovering from the call and the lasting affects it had on me. 
Any advice or on how I can do the MR?

    Yes I’ve had the letter back saying they haven’t awarded it

    You should put the request in writing stating where and why you think you should have scored those points. Don't focus too much on the assessment report or what was said in the decision letter. Doing this isn't going to get you a PIP award. If there's any obvious contradictions it's ok to point out a couple of those. Then put them to one side.
