It won’t be on the actual form. It should be in the letter you received with the form.

Good morning, this morning I received a message from the DWP department stating I haven't sent off all they haven't received my how your disability affects you form they really don't know what they are doing do  they ? I've only just had the form sent to me on this Saturday the 29th of April so don't need check on their system to see when I applied for it ? Will give them a ring  9:00 this morning .. and the way they sent the documents in the envelope was terrible the adress was handwritten in red pen and it was written right in the corner terrible when I open the envelope there was no date on the form to tell me how long I had before I could send it off and there was no separate disability AIDS leaflet like they said they would send me ..🤔

The text you received this morning will have been automatically generated as a reminder. When did you ring to start your claim?

I rang them on the 13th of April to start to my pip claim

Then your form will probably need to be returned a month later, so about 13th May. It took a couple of weeks for you to receive the form, not unusual. This explains why you received that reminder text.

When you ring ask for some extra time to return it. I note that this is the 2nd you’ve applied as you were refused the last time.

Please make sure when you fill out the form that you give a couple of real world examples of exactly what happened the last time you attempted each activity that applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.

You can also ask those that know you well to write a supporting letter for you. Make sure they include the descriptors that you think apply to you.

If you do the same as you did previously then you risk being refused again. For this reason I would usually advise that you challenge a decision, rather than a new claim.

Thankyou poppy, also can you please tell me if I have to sign and put my reference number on every sheet of the forms plus my evidence because I have so many to send to them ??

No, you don't need to sign every sheet of the form. You need to put your name an National Insurance (NI) number (not reference number) on everything else you send.

Remember that with evidence less is often more. Sending several pieces of evidence all saying the same thing isn't helpful.

Omgoodness thank you poppy so in regards to all my evidence I have to sign and put my national insurance number on every piece of evidence holy dear that's going to take a while lol 😊

You don't need to sign them, just your name will be fine as well as your NI number. It seems like you're planning on sending in quite a large amount of evidence, is that correct?

I remember you applied for PIP previously and been refused, is that also correct?

Hi - no, you just put your name & National Insurance number on any extra pages you've added, & on each page of any evidence you're sending. Do check your evidence to make sure there's nothing just repeating a diagnosis for example, or contradictory to what you've put in your claim form. As poppy says, 'less is more.'

If you put your evidence into a Microsoft Word document and print @Tina12, you could add your name and national insurance number as a header. That would mean it would automatically add your name and national insurance to each page once you have done it once! Just thinking this might potentially save you some time and effort  :)
