I worried I send my pip foam forgot to sign out a date next to the signature. What do I do? I’m panicking as I suffer with anxiety. I can’t do face-to-face contacts as I shake my knees shake. I’ve got really bad nerves

I wouldn't worry too much about the date. If there's a problem then they will return it to you for you to put a date and return it. There would be more of an issue if it wasn't signed. Most assessments are be phone, some are face to face or video. If it is face to face then you can ring to ask for a telephone assessment.0

Hello @peachy1111
Welcome to the community! As Poppy said, if they see it as an issue, they'll send it back for you to date it. I've sent forms and put the date the American way around before, no idea why. And I once dated a form in 2020 as 2013
I'm so sorry you have such bad nerves, do you have support around you? We are always here if you need to talk or vent0
