Hi @minion46 - & welcome to the community. This sounds like this might be a review form you completed from what you've said. I'm sorry for the disabilities you have, & how your upcoming assessment is causing you stress.
Please try not to worry, as medical input is likely not needed as your diagnoses will not be in question. You should also have the opportunity to mention that you're aware that there were gaps in your form, which you're hoping to now mention.
When then asked questions with your assessment try & give a couple of recent, detailed examples as to the difficulty you face for each applicable descriptor, i.e. when did it happen, where, what exactly happened, did anyone see this, & were there any consequences to attempting/doing an activity such as fatigue?
Say if you can't do an activity 'reliably,' i.e. safely, to an acceptable standard, repeat as often as one would reasonably expect, or if it takes you much longer than someone without a disability.
This will show your individual response to each applicable descriptor, & should go a long way to show exactly the difficulties you face with these activities of daily living, & mobility if applicable. Hoping this helps, & wishing you well. You can do this.

It wasn’t a review form it was the form to say how my disability’s affect me

There’s very likely a lot of people that don’t fill out the forms properly but it doesn’t mean a refusal.

All you can do is give as much information as possible during the assessment and try not to answer any questions with just a yes or no.
