Some questions about a PIP review

Hi there all.  I've just received the PIP renewal form.  It's the first time I've seen it so I'm super anxious.  I've just got a couple of questions if anyone can help.  Thanks.

I had the original assessment recorded.  Can I request that again and get a CD?
Can avoid a F2F by also requesting a phone assessment?

Then there's the issue of evidence which is really thorny.  Of course this time I will print two copies of anything I send them - 1 for them and 1 for me, but I can't remember what letters I sent last time.  It does say don't send us stuff you've sent before, but if I can't remember what letters I used, should I just send what I think is most relevant now?  I have no "new" evidence to send (Thanks Covid).

Oh - the prescription list - I can send either a scanned and printed version of that.  Should I do this, or send them an original copy?

    You need to treat the review form as though you are making a new claim. Never just put "No Change" onto the form because that does not explain the problems you are having now. You can say that there has been no change then start to explain in detaile the problems you are having now doing the PIP activities that apply to you Reliably (Safely, repeatedly, in a timely manor and to an acceptable standard) If there is not enough room on the form to add your detailed verbal evidence then you can write or type and print it off and add seperate sheets of paper and reference those sheets of paper on your form (see additional sheet page 3 of 23) in the activity section. Add at the top of all additional sheets your Name and National insurance number. Also do this for any supporting medical evidence. Only send copies of medical evidence.
    For evidence then you can resquest a supporting letter from someone who sees you often or cares for you and ask them to detail out the problems you have and the help you need and why you need that help.
    You can ask you GP Surgery for a copy of your summary care record and include this if it supports your claim/review.
    You can keep a 7 day disability/mental health diary to show the problems you have and the help you need according to the activities descriptors.
    The best evidence to include is your verbal evidence describing in detail the problems you have now because of your condition or conditions.

    Thanks Nasturtium.  Thing is, I'm a bit closer to the deadline than I initially indicated.  All your advice, whilst good (I am treating it as a new assessment), I've already found on this site and have incorporated into reasonably detailed answers almost ready to print.  I've got the 2 week extension because they sent it late - as in ridiculously late.  I anticipate posting soon.

    It's just this time there isn't an obvious place to request things like phone appointments (to hell with F2F, if possible) and getting a recorded assessment.  I think I'll wangle these requests into an additional sheet somewhere unless directed NOT to do this.  I'm guessing "treat as new claim" = send them any evidence you want because too much / evidence they already have is better than not enough evidence?  This last one is really awkward.  I've no idea what to do.
