I've just received my medical report so i will go through that and get the CRMR1 completed.

As for the medical report, I'm just reading it now and there are numerous errors and well invented things which i didn't include in the uc50 form or in the wca.

I was under the impression that the medical report would show any points that had been awarded from the various descriptors? I can't see anything where points have been awarded.

The descriptors where i was hoping to get points i have this on the report:

"I have considered the possible UC activity outcomes and my advice is that the following apply:

    Lower limb - activity outcomes

Mobilising unaided by another person with or without a walking stick, manual wheelchair or other aid if such aid is normally, or could reasonably be, worn or used  Activity 1

    Standing and sitting   Activity 2
We          none of the above apply
Sd            none of the above apply

So I'm guessing no point awarded?
If you can explain what the above actually means it would be helpful.

There will not be any points awarded in any assessment report because only a decision maker can award any points. The report will indicate recommendations.

For this one here, is there any other information? i would expect there to be some more information.

Activity 2 for LCW descriptors they have advised that none of them apply.

I wouldn't expect the report to state what you said verbatim. It will be their opinion based on everything you wrote in the form, what you said during the assessment and any other evidence you sent.

In my last WCA report it specifically states at the back of the report their recommendation (mobilising) Support Group/LCWRA and the reasons why they recommend this

Please also note that for LCWRA it's not about scoring points. You just need to satisfy at least one of the descriptors the majority of the time.

    Thanks

This is exactly how it looks:

    Medical opinion - physical
    -----------------------------------------------------------
I have considered the possible UC activity outcomes and my advice is that the following apply:

    Lower limb - activity outcomes

Mobilising unaided by another person with or without a walking stick, manual wheelchair or other aid if such aid is normally, or could reasonably be, worn or used  Activity 1
    -----------------------------------------------------------
We    None of the above apply

    Standing and sitting            Activity 2
    -----------------------------------------------------------
Sd      None of the above apply

Lower Limb - supporting medical evidence
Prominent features of functional ability relevant to daily living
    -----------------------------------------------------------
    Is stated that:

There is a list of things i can do regarding walking distance etc

Towards the back of the report there is this:

Limited capability for work related activity
Evidence to support the opinion that the person does not meet any of the descriptors for limited capability for work related activity

    Terminally ill/chemo/pregnancy - No

    Substantial mental or physical risk:
The evidence does not suggest the client has a condition which means there would be a substantial risk to the mental or physical health of any person if they were found capable of work or work related activity.

OK thanks, that makes sense now you've laid it out that way, rather than the previous comment. I can see it says the mobilising activity doesn't apply (so they think) .

Sorry i'm a little slow today, not feeling great the past few days.

Now just concentrate on the descriptors you think do apply and why. Please don't forget to mention why you will not be able to self propel a manual wheelchair because this also applies to mobilising.

I have completed the CRMR1 form and just about to post it but I'm unsure of the address. It states to use the address at the top of my decision letter. I only received a UC journal entry decision and a UCD83 pdf file.
The address at the top of the UCD83 is:

FREEPOST DWP, Universal Credit Full Service

Is that the correct address, just there's not even a postcode?

    That is correct.

Make a copy of the form for your record.
