It's part of UC so she will be entitled to the extra £390.06/month which will be part of her maximum entitlement.

It's paid from the 4th month after she first reported her health condition/sent her first fit note. When it's paid from exactly will depend on the date she reported her health condition/sent first fit note and what date she usually receives her money each month.

I know i'm hopeless but does that mena she will just get £390.06 for  LCWRA and no more.

It means she'll be entitled to the LCWRA element plus what ever elements she's claiming now.
If she's single, without rent costs her UC will be standard allowance £292.11 + LCWRA element £390.06/month. Total amount £682.17/month (before deductions, if there is any)
