It would have been the UC50 form for the work capability assessment. If found to have LCWRA you will be entitled to extra money from the 4th month after you reported your health condition. Providing you sent a fit note within 7 days of that and continued to send fit notes without any gaps until a decision was made on your claim.
See link for information about the work capability assessment. https://www.gov.uk/health-conditions-disability-universal-creditThis link says ESA but the process is exactly the same for UC. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/esa-medical-assessment/
You will likely need an assessment because most people have them. They are mostly done by telephone but some are face to face or video call.I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying. If i see a question that i know the answer to i will try my best to help. -

Hello @25764
Welcome to the community! How are you keeping today?
Like others have commented above, it was probably a UC50 form. How did you find filling it in?Hannah - She / Her
