Asking for a 10 year award/ ongoing award

I've been in receipt of Dla/PIP now for 12 years.im 60 years of age .
My numerous conditions are all chronic and detoirating all the time.
Over 12 years my award has gone from just enhanced mobility and nothing for living to enhanced mobility and standard living. 
However my award duration has steadily gone down from 4 years to 3 years to now 2 years.
I've just received new review forms and wonder at the end of the form it's worth asking/ putting the case forward for an ongoing award / 10 years  or will the assessor take this as me being cheeky?
Also because of severe arthritis in my hands I am having to ask someone else to fill the form in for me, do they fill it in as though it's me writing it or do they state that for instance Mr A struggles with this activity?
There doesn't seem to be anywhere on the form for someone to sign to say they have filled it in on my behalf.

    You can ask in the extra information part of the form, they might refuse but you aren’t the only person who’s asked for an ongoing award. About someone else helping you, they’re only really interested in you personally signing. Again, make a note of this in the extra information box.
