Pip assessment phone call

Hi everyone,
this is my first post and hope that I’m not boring anyone, I have my pip assessment on the 9th of august and I’m just getting my paperwork sorted, I applied to get my gp records from my doctors and was emailed to say because of Covid they have involved a 3rd party to generate this and it will cost me £40, (which I reluctantly paid). I managed to also get my hospital records free of charge by the way, so anyway, my ailments are, severe back pain with sciatica affecting my groin (had for as long as I can remember) depression disorder (diagnosed 2014) , bilateral tinnitus (diagnosed 2015) pure hypercholesterolemia (diagnosed 2010) . Checking my GP records / hospital records ,I noticed something on my hospital records that after a referral from my GP for an X-ray and attended, my results from that were that I was clinically diagnosed as having Lumber Spondylosis , 😳 but they sent this confirmation letter to the wrong GP, same building but wrong doctor so this has never been on my GP records , I’ve been on opioid pain killers for 13 years with literally no more than 6 visits to physiotherapy, I had to go to a walk in center of a weekend because of my back issues and after examining me she decided it could be the onset of shingles?, my question is do you think I should mention this to the so called health professional on my assessment ? I’ve already approached my GP about it and he’s not got back to me yet

If you’re asking about whether you mention the shingles then I would say no because it’s not a long term health condition.If you’re asking should you mention the Lumber Spondylosis diagnosis then yes you can. However PIP isn’t about a diagnosis. What they look at it how your conditions affect you against the 12 PIP activities.0
