Medication review outcome inform UC LCWRA

Had my medication review with my doctors after 4 and half years.
They are increasing my dosage and referring me to the mental health team for CBT.
I’m on LCWRA do I need to update UC of this review outcome on my journal?
I’m also awaiting a LCWRA reassessment review. I sent the forms back in July and on the forms it has my previous medication info but not this new info as I have only just had a review now and been advised of the increase in dosage and referred to mental health team for CBT so do I need to inform the assessment people?
Not sure how long I’m waiting for my assessment review with LCWRA.

Thank you I’m still waiting for the evidence to show up on my nhs record as the lady is referring it back to my doctor so the changes can be made or can I just tell them without it and that they can contact my doctor?Also do I need to update UC on my journal?0

I would wait until you have the actual evidence. If an assessment is needed and you don't receive it by then you can just tell them during the assessment. You don't need to update UC on your journal.

do you think assessment people will contact my doctors?Not sure if they have already spoken to my doctors or request info as The lady was asking me a few personal questions like if I leave the house, how often I have suicidal thoughts etc but not sure if this is just because of my medication review very strange.0 -
It’s been 5 weeks scince returned forms and heard nothing from them even been checking my uc journal to see if I have anything but nothing0

It's rare for them to contact your GP or anyone else. If they are contacted they will be sent the UC113 form to fill in and return.

i just found a letter from my doctors ! which explains my health conditions it’s for my housing for me to be moved in more suitable accommodation but explains how my health conditions effect me!I didn’t send this with my assessment forms
can I email this over to them now?
