Hi @welly123A - & welcome to the community. I'm sorry about your PIP decision. You can do what's called a Mandatory Reconsideration where another decision maker will look at all of your claim again.
This is best put in writing rather than ringing up. As far as PIP goes, remember it's not about any diagnosis, but rather how your disability affects certain activities of daily living &/mobility. Have a look again at the activities/descriptors that are looked at with PIP:
    https://www.cambridgeshire.gov.uk/asset-library/Personal-Independence-Payment-Descriptors-and-Scores-April-2021.pdf
You may not have done so with your claim form, so now give a couple of recent, detailed examples as to the difficulty you face for each applicable descriptor, i.e. when did it happen, where, what happened, did anyone see this, & were there any consequences to attempting/doing an activity?
Say if you can't do an activity 'reliably,' i.e. safely, to an acceptable standard, repeat as often as one would reasonably expect, or if it takes you much longer than someone without a disability. See this mentioned at the end of the link above.
Ensure you put your name & National Insurance number on each page. Keep a copy, & get a free Certificate of Posting from your Post Office when sending it off.

Hi thank you for your response they have scored me 4 on my moving around it says you can stand and the move more than 50 metre but no more than 200 metres either aided or unaided Them recently got a letter from the doctor that says needs to rest after 20 metre off walking independent transferring between seats does this mean I could get better points if I send this in ?

Yes, you could send your Dr's letter, tho perhaps this is something you told your Dr. But using this activity as an example, with your Mandatory Reconsideration could you give a couple of recent examples explaining why you need a rest after walking 20m.? Is this because it say causes pain, you become breathless or fatigued? Are you likely to fall; has this happened, & how often? Could you repeat walking this distance again or would you be unable to do this after you rested; how long do you have to stop for? Does it take you longer than someone without your disability to walk a certain distance?
These are the sort of details that would illustrate the difficulties you face, & only you know what those are.
You would need another 4 points or more for the standard award, & another 8 or more for the enhanced. If you also gained some points from the 'Going out' activity, which is not to do with any physical problems, these would be added to those for the 'Moving around' activity.

Transferring between seats is not considered for the moving around part of mobility. In my opinion that letter you have from your doctor isn't the best evidence to send, if that's all it says. You need to give a couple of real world examples as mentioned above.

This is for getting another decision maker to look at your claim again with your written Mandatory Reconsideration. You need to give a couple of recent examples as to your difficulty, e.g. where were you walking, when was this, what exactly happened, why did you have difficulty, was anyone with you, how did the activity leave you feeling afterwards?
Say why you become out of breath, how far had you walked before you had to stop because of being out of breath, where was the pain, why do you have pain in your leg(s), where was the pain there? Could you walk as far again after resting, how long did you have to stop for? These are the details needed which you just need to describe in your own words. You don't need to speak with your Drs, just write down your experience with your Mandatory Reconsideration letter giving relevant details such as above.

Good idea to get someone to help you write your Mandatory Reconsideration. Perhaps you can show them the sort of examples above that may be helpful.
You have to write about how you were at the time of your assessment, so a new medication may not matter.
