    I have a question what is this form used for and why in regards to UC as I think something is not right my husband had WCA ASSESSMENT Last year and what I understand they did not passed this to ESa until now, and they removed SDP ? 
    He was told he is not getting LCwRW when his outcome was special circumstances and 14 months reassessment for WCapability. 
    We make a claim for ESA got letter that he is eligible for both ESA IR and C but he only gets C one also they letter sound like they did not know about his WCA when decision was in October and we send claim after he was told he will not get any help due to health conditions as it's only LCW awarded for him, we send claim after his sick pay ended for Esa. 
    This information about the Mitigration we found out as I made SAR REQUEST to DWP. 
    can anyone explain what is this doc.attached and why noone inform us about this as this also affected our c.tax support which was removed backwards so I have no idea what they done on uc claim and they say nothing about it.

    You shouldn't put up personal details. Lot of scammers out there.

    I would encourage you to reach out to Citizen's Advice. They have some information pages on benefits that you may find helpful to read, but I think speaking to one of their advisors would give you more clarity :)
