PIP review form not arrived, twice

5 weeks ago I was sent a text to say my review had started and to expect my form in 7 to 10 days and to not call for 2 weeks. I waited and nothing arrived so I called and it was resent. 2 weeks later, again nothing. I called again added another address for it to be sent to and I'm 1 week in and still waiting. In the meantime I've received a letter saying I've not returned my form. I'm getting really concerned now, no forms have appeared from the first 2 that were sent out and they don't send them by email. What's happening? Will they stop my money? Can I pick a form up from somewhere?

You will need to ring PIP again. Unfortuntely you will not be able to pick the form up from anywhere or download it online.

Finally received the form but the information section that they say is included is not in the envelope. The spaces to write your info in seem very small. Do I fill it in like it's a new claim with lots of info. I have multiple disabilities so the spaces are too small to fit it all in. Or do I just sat no change and provide 1 sentence for each disability0

There's not much in the way of information notes for a review. Perhaps you mean this: https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/756549/ar1-notes-pip-award-review-information-to-help-you.pdfHowever, the examples given are not great, as they're so short (& the assessor may not be able to see your previous claim form).So, treat it as if it were an initial claim form, & you can just continue your answers by adding pages at the end of the review. Just say which question it's a continuation from, & put your name & National Insurance number on each of these additional pages.0
