The PIP change of circumstances form is pretty much the shortened review form, have you already received this?

If you're wanting to report a change of circumstances, you can just call the DWP enquiry line;

PIP enquiry line
Telephone: 0800 121 4433
Textphone: 0800 121 4493
Relay UK (if you cannot hear or speak on the phone): 18001 then 0800 121 4433

Usually when reporting a change of circumstances they send the PIP2 form, north AR1 review form.

Its no different to the very first time you claimed PIP. Make sure treat it as a new claim and put as much information as possible, without telling your life story.

Also include a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you.
