The PIP change of circumstances form is pretty much the shortened review form, have you already received this?

Usually when reporting a change of circumstances they send the PIP2 form, north AR1 review form.
There’s a guide here to filling it out https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/help-with-your-claim/fill-in-form-pip/

Its no different to the very first time you claimed PIP. Make sure treat it as a new claim and put as much information as possible, without telling your life story.

Also include a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you.

There’s also lots of information on many threads in our PIP section.
I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.
If i see a question that i know the answer to i will try my best to help.
