how to fill in in the pip2 form for change of circumstances

If you're wanting to report a change of circumstances, you can just call the DWP enquiry line;
PIP enquiry line
Telephone: 0800 121 4433
Textphone: 0800 121 4493

Its no different to the very first time you claimed PIP. Make sure treat it as a new claim and put as much information as possible, without telling your life story.
Also include a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you.There’s also lots of information on many threads in our PIP section.1
