PIP review awards seemingly being made without asking for up to date evidence.

Been mulling this over for a year or two now.
When I was forced onto PIP from DLA (High Care & Mobility) back in 2017 I completed the claim form the best way I could. The result was that I failed to get any points - so no PIP for me!
I then decided to challenge the decision on the basis that I have not got improved mobility and still need care at the same level as it was for DLA. Yes I know that they are both different benefits assessed in different ways.
Anyhow with the help of my GP he wrote a report covering all of the PIP descriptors pointing out the evidence that was held at the surgery.
I went from my high DLA award to nothing for PIP then to Enhanced for both for 3 years.
Then in 2019 I received a letter from the DWP telling me that my PIP had been reviewed and that it would stay the same but for a further 10 years.
How was this possible given my PIP history and having no up to date evidence?
Under the 'soft touch' in 2029 surely they would not just base a decision on evidence from 2017?
The reason I ask is it good that I continue to gather relevant evidence from now until 2029 when my award is up for another review - how would they know if there has been any improvement?
