PIP forms, evidence & history

Hi
Unfortunately because of my health I’ve had to start a claim with PIP, I’ve done everything to avoid making a claim, but my health has deteriorated over the last year, I have Asthma, Adult Onset Still’s Disease, I have had 21 operations on my vocal chords, I have had 1 x hip replacement surgery on my left leg and 2 x hip replacement surgery on my right leg, I have also got other problems with my Lungs, to which I am awaiting diagnoses for. So I have a couple of questions that I need a little help and advice with:
1. With regards to evidence, what information do I need to submit? Do I have to submit surgery dates? Hospital admissions? Hospital appointments? CT Scan dates? Lung Capacity test dates?
2. With regards to the history of my disabilities how far do I go back? I’m 53 and have had both my hips replaced which was caused by having Still’s Disease as a child, also I had sepsis and pneumonia last year and I am now suffering from Adult Onset Still’s disease.

I'm so sorry to hear your health hasn't been great recently. I hope you are getting support?
With evidence, Citizen's Advice has a page that you may find helpful. Getting evidence to support your PIP claim - Citizens Advice. With how far back you go. I would write it quite factual like you have here. For example, In (year) had (surgery / treatment / illness) and has (xyz) effect on my health.0 -
