I just want to share how I got a copy of the assessment form for lcwra.
I did a request for information online and about three weeks later it arrived in the post. Signed for delivery. Totally free.
It contains your original application and also you can see if the GP bothered to fill in the form DWP sent out. Mine didn't so I appealled and won.
Anyone can request information on journal messages, payment statements, anything. It's really helpful if you go from a single claim to a joint claim because you loose all the journal previously available to you.
