Try googling those codes and you'll find discussion groups like rightsnet looking at different health assessment reports

I have googled these two form sand look in rightsnet but I do not found them.
We can called them the "mysterious forms" because nobody can access them what is not transparent

No one will be able to download them because they are not available to download in the guide. I've looked for UC85 on the internet but can't find anything either.

What matters now is that you register your dissatisfaction with UC to request an explanation of the redaction plus a new copy of the assessors report. I would ring them as journal messages often go unanswered..
Perhaps the health assessment services can provide you with a copy but I don't know so ask them. Perhaps DWP is updating the forms, again no idea.
You can request mandatory reconsideration (MR) of any decision without all the details at this stage. Only once you receive an appeal 'bundle' from Tribunal services will you have full sight of all the information used by DWP to reach their decisions.

Yes, but it is always better to have all information in hand before contacting the UC department.
It is good if I know if pages are missing in the copy of the UC85 form sent to me or if some parts have not been properly filled by the healthcare professional.
Maybe someone from this forum whose English is his first language can get a copy of the UC85 and the ESA85 forms from the DWP

I'm sorry you haven't understood what we've told you.
The only people who can issue one of these "mysterious forms" is the DWP so you'll need to ask them for an explanation and request a new copy.

My first language is English but I'm a "she" not a "he" and I've already advised that the UC85 and UC85a is not available online. It would only be possible to look at one if you seen one from another person. Same applies to ESA85 and ESA85a (for anyone reading this)

I have complained to the Jobcentre because the name of the Heathcare Professional has been redacted in the copy of the UC85 form sent to me.
I have received a response from the Jobcentre saying that it is because according to Universal Credit regulations the name of the healthcare professional should be redacted.
I have asked the Jobcentre it provides me with the Universal Credit regulation which says this.
I have received a reply from the Jobcentre repeating the same thing i.e. that the name of the healthcare professional should be redacted without providing me with the Universal Credit regulation which says this.
I would like to know if you know which law or regulation says that the name of the healthcare professional who carries out the health assessment should be provided to the client so that I provide this law or regulation to the Jobcentre to convince it to send me another copy of the UC85 form without the name of the healthcare professional being redacted

There's nothing in the UC regulations 2013 that I can find that says the UC85 or UC85A should or shouldn't be signed. You can look here for yourself.
There's also nothing in the work capability assessment handbook either. You can see that here.

The issue is not whether or not the UC85 form should be signed but whether or not the name of the Healthcare Professional should redacted in the copy sent to the client
Other members of this forum in their replies to this thread have said that no so they should have some reasons to say this so there should law or regulations concerning this

Found the following link from a What They Know blog - it's a number of years old - but it may help to shine a little light on some of your concerns around missing fields/pages. It refers to ESA85s but would equally apply to UC85s.

I think that this document does not concern my thread because it concern electronic copy ESA85s but my health assessment report was not electronic but clerical

Clerical? Usually 2 reports are returned, one is digitally and the other by snail mail.
