What date exactly did you provide your first fit note?
Your assessment period being 13th July doesn't make sense because assessment periods last for 1 month every month, so for example 13th to 12th of every month. What date do you usually receive your money each month?

That means your assessment periods are 13th to 12th of every month. Your waiting period will be April/May, May/June, June/July.. LCWRA starts from 13th July to 12th August and first payment will be 19th August. You're not owed any LCWRA element. You will need to check your statement in August to make sure the element is included.

Who do I contact if the money isn't in on the 19th of August

You need to check your statement for that month to make sure it's included. If it isn't then put a message onto your journal under the payment section.
