UC50 not received
I helped a relative complete a UC50 form and we sent this off in the prepaid envelope on 16th August. I have proof of postage from the Post Office.
The form had to be back on 26th August. He is very unwell and asked me to call the assessment service to check that they had received the form. I called on 28th August to be told that this had not been received and that the referral had been sent back to UC. Can anyone tell me what will happen next and what we should do?

Hi @Lauren29, sorry this has happened to you. Were they able to give you any advice over the phone about the next steps? If not, it might be worth giving them a ring back or contacting the UC office that are dealing with your relative's claim to see if they can clear things up for you. If you explain the situation they may be able to send out a new form.

A new form isn't needed because your file can be returned to the Health Assessment Advisory Service. You'll likely be asked why you didn't return it on time but you have proof that you did post it back before the deadline and that's all the proof you need.
I'd advise you to put a message onto your journal for your work coach and tell them what's happened. Also mention you have proof that you posted it back to them.
