Reasonable adjustments...how long should it take to receive a response?

Hi everyone! I started a new job 3 weeks ago. I have been diagnosed with General anxiety disorder as well as asperger's syndrome. In a previous job, work made me so stressed and sick that I had panic attacks in the night and had to call an ambulance as I was convinced I was having a heart attack, and was subsequently signed off work for several weeks. I was not aware that reasonable adjustments could be made at work until after this time. I am since on anti-anxiety medication.

I started 3 weeks ago at this new role in IT. So far, I've found it overwhelming- the amount of things I have to do, juggle several requests at once, people chatting to me about different things when I'm already working on something, it's been stressful. Other members of the team have also been noisy, kicking a football around the office, winding each other up and shouting at each other which has been distracting. We are in a very small office.

Before and after my interview I mentioned that I have disabilities, detailed them during the onboarding process and undertook an occupational health assessment which was passed onto the company. I didn't hear anything during my first few days here, so I emailed the HR department who were aware of my disabilities and occupational health assessment along with its recommendations for reasonable adjustments. They said my manager was aware and to await a reply, but HR did remind the manager at the time.

I didn't hear anything for a further two weeks, so I emailed the manager asking about reasonable adjustments. It's been 48 hours since, and I still haven't had a reply or even an acknowledgement about this- though the manager has given me tasks to do via email.

I haven't even seen my manager in person, I barely know what she looks like- but I have other members of the team effectively managing me (on my level), so I don't know who to go to. I have no idea what to do next. It's my third week at this company and I don't want to become a nuisance or even an enemy this soon into my employment. On the other hand, I don't want my health to degrade to the extent it has before. I really need to discuss reasonable adjustments and I would feel embarrased and ashamed constantly emailing about this with no answer. Am I just being impatient?
