UC50 and LCWRA

Hello

My husband and I are now on UC, I've recently been sent a UC50 form as I have CP, anxiety and depression, mild-moderate hearing loss and I can't work. It says to attach medical evidence on the form but how exactly am I meant to do that? I have my medical history from when I was a child but I don't have any kind of treatment or therapy or physio (I was booked for physio at a different dr surgery and it was too far for me to go and they don't do home visits so I had to cancel) and if I want my hearing results I have to pay for a full hearing test, so what should I do? Also they gave me an envelope with the form, should I use that?

    Hi,

    It's not mandatory to add any evidence to the form. Don't pay for any evidence that you don't already have.

    As above, you will be asked to complete a face to face or telephone assessment if the information on the form is not enough for them to decide from that alone.

    Yes, you can use the pre-paid envelope provided. It is worth taking it to a post office and asking for Proof of Posting if possible. That gives you some proof of having posted the forms if the assessment centre say they never received them. It does not cost you anything to do that.

    I'll definitely do proof of postage and I am on PIP so I'll get a copy of my reward letter. I problem with my medical history documents is that there's not a lot to them if that makes sense and they're addressed to my mum not me (I got them digitally a few months back when we knew we were being switched over so I emailed the hospital for my complete medical history and although there's a lot of documents it's most stuff from when I was a baby and very young).

    Thank you! I'll definitely be photocopying my PIP award letter and at the moment screenshotting a few of my medical history documents from '92 etc. I sent my PIP application back in 2019 when I transferred over from DLA. I didn't even have anyone assess me they pretty much just called me and asked if anything had changed (no it hadnt) and moved me over.

    just tick the box giving them

    Permission to see your medical stuff . Iโ€™m sure on the form it tells you not to pay for anything .
