Support Group is LCWRA element for UC and you'll be entitled to this from the start of your claim. As your ESA includes the SDP I'd advise you to wait until the last minute to claim UC. You will not be worse off when you migrate but Transitional Protection erodes over time so if other elements increase or you become entitled to other elements TP decreases, until eventually it erodes completely.
When you do eventually claim you will be asked to provide a fit note but this isn't needed. Once you have access to your journal you'll need to make sure you add a message and tell them you're in the Support Group. It will take a few weeks for ESA to send your details across.

If you claim UC by phone, you will not have access to a journal.
