PIP. 1043 review form
Good morning,
Received first review form and we are surprised it isn’t like the original form.
Been told it seems similar to AR1 but possible a new version.
We can’t find anywhere to write down the name and addresses of specialists involved?
The only place I think we can add is Q13 Is there anything else?
Where have people put this information please?
In the original form we put this under Q1 About your health Professionals etc.

I’m not a professional but seen lots written on the forum.
I believe you fill them in like a normal claim form with as much detail as possible & to use extra paper for extra space to write things.0 -

Ah brilliant, that's great to hear. 😊 We always advise people to fill in review forms as if you were applying for the first time, it can save a lot of time further down the line and additional information can be added on separate pieces of paper, just make sure each one is numbered and contains your national insurance number. .
