UC migration from ESA, WCA question.

Hello,
I am migrating from child tax credits and ESA to UC. I have been in the support group since 2013. I last filled in an ESA50 form in August 2020 and I was told on the phone that I would be reassessed in September 2022, this reassessment has never happend, so I guess I am well over due one.
So I would just like to know if the form will still be the same as the ESA one? I am trying to mentally prepare myself so that it doesn't take me by surprise.
I hope someone can help.

Nightcity is correct, the forms and questions (and scoring) is the same for ESA and UC. Some wording on the forms has changed and they are updated each year but the changes are often only small. You can see a copy of the full UC50 form here:
