The best thing to do is to put your details into a benefits calculator. https://www.entitledto.co.uk/benefits-calculator/
If you work 16 hours or more per week then your ESA will end. You must make sure you claim UC before the ESA ends. By doing this you will be entitled to the LCWRA element from the start of your claim.
When claiming UC there’s no disability premiums payable. You will be entitled to SDP Transitional Protection because your ESA includes the SDP.

Severe Disability Premium transitional protection
How much Severe Disability Premium transitional protection you are entitled to will depend on any other disability premiums you get and your household's circumstances when you claimed Universal Credit.
- Single, with Limited Capability for Work Related Activity £140.97 per month
- Single, without Limited Capability for Work Related Activity £334.81 per month
- Couple, only one person entitled to Severe Disability Premium on legacy benefits, with Limited Capability for Work Related Activity in either partner's name £140.97 per month
- Couple, only one person entitled to Severe Disability Premium on legacy benefits, without Limited Capability for Work Related Activity in either partner's name £334.81 per month
- Couple, both entitled to Severe Disability Premium on legacy benefits £475.79 per month
You can get monthly additional amounts if you are entitled to severe disability premium transitional protection:
- Single and if you received the enhanced disability premium in your legacy benefits - £89.63
- Single and if you received the disability premium in your legacy benefits - £183.52

- Couple and if you received the disability premium in your legacy benefits - £262.48

is this on top of :In the 2024/25 tax year, the extra monthly amount for Universal Credit (UC) if you have limited capability for work and work-related activity (LCWRA) is

???. Plus the basic wage?

Your UC will include standard allowance, LCWRA element and SDP Transitional Protection. Assuming you are a single person.
As you’re living in Supported housing your rent will continue through housing benefit. If you move from there in the future then rent will be paid in with UC known as housing element.
Earnings of up to £404/month will be ignored. For every £1 over that amount your UC will reduce by 55p.
I do not know how much your housing benefit will be affected by working more hours.
