PIP renewal

I currently get the Daily Care rate of PIP, but, even though I get out of breath after 10 minutes, I don't get the mobility rate.
It is up for review, and I have had a few minor things change.
After a scan of my shoulders at a private hospital earlier this year, the result showed I have an inflamed tendon in my right shoulder, which is getting worse and can cause me quite a lot of pain. Also, I have scoliosis, which can cause me back pain as well. Because of this, I have become wobblier when I walk since my last review.
I have an osteopath that sees me every two months. Unfortunaately, these days, a permanent doctor is a thing of the past!
Together with my form, should I send them the x-rays, the report from a doctor and a report from my osteopath?
Lots of people say fill in the form from the point of view that it's your worst day. What sort of stuff should I put?
