You need to fill out the form as if it’s a new claim and give as much relevant information as possible.
It doesn’t matter if you haven’t had any therapy or other appointments since your last assessment. Many people continue to receive PIP and have no treatment.
Not everyone has medical evidence to send either. If there’s someone that knows you well you can ask them to write a supporting letter but make sure it’s related to the PIP descriptors.
You can also write a diary. Please also include a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.
There’s also a guide here that may help.
If you need extra time to return the form you ask ring to ask and they will give an extra couple of weeks.

Yes, I hope that I can ask the GP to help but they do not want a letter from the GP but actauly care plan or therapy plan instead. best an thank you very much

Letters from a GP are never the best to send anyway. This is because a GP will very rarely know exactly how your conditions affect you. If they know it’s because you’ve told them. They don’t spend enough of time with you to know how you manage the activities based on the PIP descriptors.
