Not sure what type of employment you are in but I would first get hold of a copy of your employers absence management policy…..it should very clearly lay out the various stages you will hit as you progress through any sickness period and especially so for long term sickness.
It will generally give the information an employee needs and importantly timescales and potential outcomes at various warning/ breach stages etc.
You are entitled too see any workplace policy on request and all policies should be easily accessible.
It might help others here to give advice if you provide a bit more information.
