so with Christmas just around the corner I have been really struggling to gather any evidence to support my uc50 form, some things I just can’t get hold of as reports to support me from hospital have not been sent to my gp as of yet to try get a copie of

I have no one to do me a letter of support

I can’t get any appointment at this time near Christmas with anyone like citizens advice or any similar agency’s

I’ve not attempted the form yet as I just feel I’m going to mess it up

Struggling to know what they want from me to support the form my talking therapist said she can’t do me any support letter only my final form she sends gp regarding risk assessment and gadq9 or what ever it is

I know people have tried to advise me on this before but I’m at my whits end with it

I just don’t know what to do and it has to be with them by 13th jan 🤷‍♀️ Nightmare

    If you don't have medical evidence then you can't send it, it's not the end of the world. Many people don't have medical evidence to support their claim.

    Some people write a diary and send that as evidence. In a previous thread you mentioned an adult son, can you ask him for a supporting letter? If you've looked at the descriptors and think at least one of them apply to you I'd advice him to make sure he focuses on that for that letter. You can see the descriptors here for LCWRA.

    I always find it easier to fill the form in a little at a time, this way it's not as overwhelming. Make sure you include a couple of recent real world examples of exactly what happened the last time you attempted each descriptor you think applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.

    I’m really struggling to even make a start with this uc 50

    I have such a lot going on at the moment and I’m struggling to focus on the questions

    My mind is just wondering around all the time

    Every time I try to focus on the questions

    I can’t focus

    Driving my self crazy

    Can’t even think how the descriptors apply to me

    🤦‍♀️

    When does the form have to be returned by? The deadline to return the form is very strict unless you have a vulnerable marker on your file.

    Did you look at the guide I posted on one of your previous threads? All I can advise is that you attempt to put something on the form and return it to them because that would be better than not returning it at all.

    form has to be there by 13th jan
    not got long to get it sorted

    tried to read it through will attempt tomorrow

    I have loud tinnitus that is very distracting to focus

    son was meant to do me a letter

    But argued he left home while I was out and it’s been a rough 2 weeks

    MKing things difficult for me all that extra worry

    You're welcome. I'm sorry to hear that. Those forms are very stressful, even for me and I don't usually let things like that get to me but they're not easy to do.

    I usually set a date when I will start the form and then I will do a little each day. You can write a diary yourself and send that with the form.

    Usually I will advise people to give as much relevant information as possible but if you're unable to do that I would advise you to just write what you can but please make sure it's relevant.

    Do you have another family member or even a friend that you trust that can come and help you with the form?

    it’s difficult with my son upping and leaving me at Xmas, I’ve not seen him since 12th dec, and he doesn’t want to see me, just took his stuff while I was out at an appointment:(

    The forms are causing me a lot of stress

    I just want them done and sent so I can concentrate on other stuff that needs doing

    I don’t have any one to help me

    Going to just have to do my best

    Like you say not at a time

    I’ve never been any good with forms

    Hey @sarah2024 I'm sorry to hear about your son. Hopefully he'll be back soon after clearing his head a bit.

    If you feel you could use some extra help with your forms, it's worth getting in touch with AdviceLocal they should be able to point you in the right direction of someone nearby.

    I’ve tried local advice places and they are all booked up till end January

    It’s one thing after another lately and I really need help completing the forms

    My son won’t be coming back

    He’s left permanently

    No where has available appointments before my form needs to be in by 13th jan

    I guess it may be a very busy time of the year for them, I'm sorry.

    I don't do great with forms, but I found (as Poppy said) it's good to set yourself a start date, then just do a bit at a time. Write down some notes first over a couple of days when you remember (I always use bullet points) for important things you need to mention and that should hopefully take a bit of the pressure off. ☺️

    hi does anyone know if it’s worth trying to get hold of a patient summary for my uc50 form

    Or is it pointless do they contact gp anyway ?

    And if I did ask the gp reception about it

    How long does it take to get hold of it ?

    Is it a long process

    They very rarely contact GP's.

    It should be quite simple for the GP receptionist to print it out for you there and then.
