A claim will need to be stated in individual names. A linking code would be sent to your wife on her claim and she would use that to link both claims together. They will already know that she's your appointee and this would apply to UC without needing to do anything else.
Usually an appointee is needed when you first claim but this can be done as a telephone call.
If you're in the Support Group for ESA you'll automatically be entitled to the LCWRA element from the start of your claim. It may take a few weeks to update your claim with this information but you will not need to provide any fit notes, even if they ask you, which they shouldn't do.
Your wife needs to make sure she reports being your carer when she fills out the form. The carers element will then be included with your UC. Carers allowance will be deducted in full. Payments for that will then continue as normal.
Income Related ESA and Housing benefit (if you're claiming it) will continue for 2 weeks once a claim is submitted.

Then it will be Limited capability for work (LCW) for you. If your ESA claim started after April 2017 then you will not receive any extra money for this group, same as it is for ESA.
Is there any reason why your wife doesn't claim carers allowance for looking after you? I'm sure she looks after you for at least 35 hours per week. If you are the main claimant of the ESA then your wife would also not be receiving any NI credits so she's missing out on that too.
You don't need to claim carers allowance to be entitled to carers element of UC BUT if she claims carers allowance she can ask for it to be backdated for 3 months, so I'd advise her to do that ASAP.
As a carer when claiming UC she will have no work commitments.
