I’m really struggling to fill out my medical housing priority form - had it nearly 2 weeks and still can’t manage it

Hi @letitbe sorry to hear this 😔 is there anything in-particular on the form that's making it difficult? Do you have a friend or family member that you could talk it through with?

don’t have any family here Holly and I’ve pushed all my friends away so no one really bothers with me anymore . I managed to fill in my name and DOB and then just couldn’t do anymore . I need to somehow do it this week apcoz I’ve had it nearly 2 weeks already .

Oh @letitbe I'm really sorry. I've had a look at the form and there looks to be 2 key area's that you'll need to pop most of the detail -
- "Please give details of why the condition is made worse by your present home"
Could you start to bullet point a list of the problems with your home which are making your condition worse/less manageable? Maybe begin with the things that cause you the most issues as top priority.
- "Please give details of your medication and any treatment you receive or have received, including dates of the last treatment and details of your current prescription(s)".
Have you got the information together on your medical history/reached out to your GP if needed? If not, it might be worth contacting them first thing Monday for anything you might be missing that will support this section.

It might be worth contacting your local authority tomorrow too, as they do offer some support with completing forms. Link - Find your local authority (GOV.UK).

thx so much for doing this Holly I really appreciate it - I just get so overwhelmed and stressed and head hurts when I have to do these things and I end up just going back to bed .
I’ll def look an read the link you’ve attached . I did have a look through the form and you’re right, the 2 areas you’ve mentioned seem to be vital - I’ll have to really try and get it right .
I do have supporting letters and I did get a gp print out of my medication last week so will include these . Thx again holly0 -

I completely get it, I can be the same with forms - can get so overwhelming your mind goes blank. There's nothing worse is there! 🙈
Might be a good idea to take a little break, maybe get some fresh air. I hope you get on with it ok. 🙂

thanks holly - I’ve only managed my name and DOB today and got a headache so I’m in bed all day . I’ll have to try and get it done this week . Thx 🙏

If you are struggling with physically filling out the form Citizens Advice can help you fill the forms out.

I went to agency to ask for help but they were so busy told me to queue in morning from 6am get a ticket at 8am then to come back at 10am .
I’ll have to get it done this week somehow otherwise the Council will close my case I’m sure .0 -
Hey @letitbe, if you inform the council you are having difficulty filling in the form you could try either asking if they have any support for form filling or if they can provide an extension.

a friend has been trying to help me on the phone Jimm , I’ve managed to get some done but terrible headache now so need a rest from it all
