Connect to work scheme.

So,I've been reading about this 'Connect to Work' scheme, being rolled out from next week in West London. It's aimed at disabled people who are not in work. The article started off by implying that unemployed disabled people would be 'targeted',but further along,it says that you need to apply to join it. I must admit I'm a bit anxious as I live in West London, and am quite badly disabled with osteoarthritis and rheumatoid arthritis, currently awaiting a hospital appointment to be approved for the waiting list for a hip replacement operation. The last thing I need at this current time is being pushed into looking for work, when truth to be told,I am struggling to get through the day a lot of the time. Does anyone out there know anything about this scheme? Is it voluntary? Or can I expect to be contacted by my local Jobcentre and pushed into looking for work?
