It shouldn't be too late to request the full assessment report. DWP should still have it on file so it would be simple for them to print and send a copy to you.
When you make the request be very specific about what it is you want.

h @Kimi87 thank you so much I did get sent the same award letter when asked, that was a couple of weeks ago, with it being so long since my last award 2017 what should I state on the request form as I did ask for the report last time and then only sent the same award letter issued 10 years ago, thanks so much again I really appreciate your help.

You can ask for a copy of the full assessment report (PA4).

hi @Kimi87 I've just had to do another form as I sent one but I deduct quote the PA4 documentation as I just asked for the full report, so I just completed another form with the directions you gave for PA4 report. Thank you again!

As Kimi seems to have intimated, apparently you can still ask for your PA4 form from 2017. Again I wonder why this is needed? The assessor's report is only part of what a Decision Maker looks at (& whilst they often do go with the Health care Professional's recommendations, in itself it doesn't matter). They look at all the evidence you've sent in too.
Your decision letter will have said where points have been allocated, so you simply don't need the PA4 form, as the Decision Maker may have either gone along with this, or not. It's your decision letter that truly matters!

hi @chiarieds I didn't request this information when my award was granted so that's why I'm asking as I do have the award letter points and reasons but never had the information relating to assessors report and DM report regards my award granted in 2017

I'm worried now that asking for this report of PA4 will go against me I'm sorry I'm not up to date with all the changes and also just having to migrate to UC from ESA SG which I was awarded indefinitely at tribunal in 2014

Asking for a copy of tbe report won't count against you in any way.
You are entitled to request any data the DWP hold about you, that's why the online service exists.
