Pip help

On my first pip application (5 years ago)I was refused pip then after the Mr I was awarded enhanced on both parts, I'm going through my review at the moment and had my phone assement ,I recieved the assessors report and it looks like it's a 0 again however on this report he states same as last assement nothing has changed . Would the assessor be looking at my first report and not the Mr decision of full award and would the decision maker see that I have full pip ..I wrote that nothing has changed I'm so confused by this would the decision maker also question this as to why the assessed is referring to the very first report and not the Mr decions that gave me full award ..I'm waiting on a decision and I'm so worried I'm loosing my award can anyone give me any thought on this please
