PIP and DLA use different criteria.
PIP is about functionallity and not your conditions. Medical evidence is usfull only to bacck up how your condition affects your functionality.
First thing is to understand the points system, descriptors and the criteria for an award. Bset starting place is the B&W self test

You do not need to squash everything into the little boxes, you can add sheets at the back of the form with your name and NIN and just reference in the box to the added sheet.
If you need help with an activity or you use an aid ie. a walking stick say why you need it and what happens without it
Make use of the reliability factors and apply to every descriptor

To an acceptable standard
In a reasonable time frame
To qualify something must be in occurance for the majority of time
