Making a PIP complaint

Hi all I have rang the pip today to complain about the computers been down and I think I had a unfair pip assessment.  Next step is to complete there complaints form which I will be doing.  I will also be asking for a copy of the report.  I am really stressed out and all this is causing me more seizures.
