Paper based decisions

After reading numerous posts on forums. Read that some people are getting paper based decisions if they got higher rate DLA, is this true?
We are still awaiting for pip forms and doctor appointments so we can get doctors notes and various medical records.
My wife allowed DWP to read her original claim for DLA back in 2001 and has updated them ever since.
Do we need any more evidence?

I can't comment on how much evidence to send in. Maybe some people do not get a face to face assessment when moving from DLA to PIP.
However I was on DLA - High Mobility & Care indefinitely, but when I moved to PIP I had a face to face and as a result the DWP awarded me nothing! I lost everything including the Motability car.
I seem to remember the fact that approx.46% of those being transferred over had a reduced award or lost it altogether.
