Why haven't I been sent a UC50 form yet?

Hello,

I've been submitting sick notes for my anxiety & depression since June of this year & I still haven't been sent a UC50 form. I've asked Universal Credit multiple times & the excuse I'm given is that they are waiting for a referral for the UC50 form to be sent & no time scale is currently in place for this. Also I've been told that it usually gets sent automatically after sending sick notes in for ten weeks which I have far surpassed now & quite frankly I'm fed up. Each time I'm given a different excuse. Please can someone give me some advice?

    Hello, 

    Thankyou for your help. I rang UC today & kicked up a fuss. They have now sent my referral across & I should be receiving the UC50 form shortly.

    Hopefully your phone call will do it. But if the UC50 is still not forthcoming, I would recommend asking your Work Coach to request it, as it's them that should have done it in the first place.
