Hi @LRI and welcome to the community! Did you scan a copy of your report? You could print it out and sign it. Alternatively, you could ring DWP to see what they say. But you may have to fill out a new claim form.

I kept a copy of it ,that’s how I realised I hadn’t signed it.Ive tried to call PIP helpline but was left on hold for over 30 mins
It had to be sent back by 2/10/19.
only leaves 2 weeks to get another one and send it back again?

Take a copy of the copy, sign it and send it. No need to ring to ask for another form.

Hi @LRI. Have you managed to send off the copy? It might be worth including a brief letter explaining what's happened.

I rang them today
They have put a note on the computer details on my file and will send the form back for me to sign.
