hi can some one tell me what di I out in the review form about hospital and physiotherapist as I don’t go to them now and I don’t have discharge letters what do I put as don’t have

    Hi,

    You just need to put that you no longer attend those appointments. If you're talking about the activity managing and monitoring a health condition then no points will be scored here for attending appointments anyway. Points are only scored in this activity if you have therapy in your own home, that must have been recommended by a health professional.

    The most you could score here will be one point, if you need either assistance/prompting or you use an aid/appliance to take medication.
    I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.
    If i see a question that i know the answer to i will try my best to help.
